Understanding Fetal Medicine & High-Risk Pregnancy

Dr. Kunda Shahane explains what fetal medicine involves and when specialist care is needed during pregnancy.

Dr. Kunda Shahane · Mayflower Fetal Medicine Centre, Nagpur

What Is a High-Risk Pregnancy?

A high-risk pregnancy is one where the mother, the baby, or both face an increased chance of complications compared to a typical pregnancy. This does not mean that a complication will definitely occur — it means that closer monitoring and specialist input are needed to identify problems early and act on them promptly.

High-risk pregnancies require a different level of surveillance than routine obstetric care. This is where a fetal medicine specialist — not just a general OB-GYN — makes a critical difference. At Mayflower Fetal Medicine Centre, Dr. Kunda Shahane evaluates the fetal condition using the most advanced tools available, giving you and your primary obstetrician the information needed to make the best decisions for your pregnancy.

Important to know: Being told your pregnancy is high-risk can feel frightening. But in most cases, specialist monitoring — done early and consistently — allows the pregnancy to be managed safely and successfully. Most high-risk pregnancies, when properly monitored, result in healthy outcomes. Dr. Kunda Shahane's role is to give you that monitoring and that clarity.

What Dr. Kunda Shahane Evaluates in Your Pregnancy

High-risk pregnancy monitoring at Mayflower is not a single scan — it is a structured, ongoing programme tailored to your specific risk factors.

1
Detailed Fetal Anatomy Survey
Complete 20-week anomaly scan covering all fetal organ systems — brain, heart, spine, face, abdomen, kidneys, limbs. Using ISUOG-standard protocol on GE Voluson.
2
Fetal Echocardiography
Detailed fetal heart assessment with 4-chamber view, outflow tracts, aortic arch, and AI-assisted STIC technology. Recommended for all high-risk pregnancies.
3
Doppler Studies
Umbilical artery, middle cerebral artery (MCA), uterine artery, and ductus venosus Doppler to assess placental function, fetal circulation, and signs of compromise.
4
Serial Growth Scans
Regular measurement of fetal head, abdomen, femur, and estimated weight. Growth velocity tracked over time to detect IUGR (intrauterine growth restriction) early.
5
Amniotic Fluid Assessment
Amniotic Fluid Index (AFI) and Single Deepest Pocket (SDP) measured at every visit. Polyhydramnios or oligohydramnios can indicate fetal or placental problems.
6
Fetal Neurosonography
Detailed assessment of fetal brain — ventricles, cortex, cerebellum, posterior fossa — when brain abnormality is suspected or previous history is relevant.
7
First Trimester Screening
NT scan at 11–14 weeks with FMF-standard protocol, nasal bone, ductus venosus, tricuspid flow, PAPP-A, free βhCG — for chromosomal and pre-eclampsia risk assessment.
8
Genetic Counseling & Testing
NIPT counseling, result interpretation, and invasive testing (amniocentesis, CVS) when indicated — with compassionate counseling throughout the process.
9
Cervical Length Monitoring
Transvaginal cervical length measurement at 18–24 weeks for women at risk of preterm birth — a short cervix can be treated with progesterone or cerclage.
10
Delivery Planning Consultation
For complex cases, Dr. Kunda Shahane provides a specialist opinion on optimal timing and mode of delivery, coordinating with the patient's primary obstetrician and neonatal team.

Please bring all previous scan reports, blood test results, your antenatal card, and any specialist letters. Our team will review these before the consultation.
2
First Visit
Clinical History & Risk Assessment

3
During the Scan
Ultrasound Assessment on GE Voluson
The scan is performed by Dr. Kunda Shahane herself using the GE Voluson Signature Expert. She will explain what she is looking at as the scan proceeds. You are welcome to have one companion present.
4
After the Scan

High-Risk Pregnancy — Frequently Asked Questions

What makes a pregnancy high-risk?
A pregnancy is considered high-risk when maternal or fetal factors increase the likelihood of complications. These include pre-existing conditions such as diabetes, hypertension, thyroid disease, or heart disease; pregnancy complications such as gestational diabetes, pre-eclampsia, IUGR, or placenta praevia; fetal concerns such as structural anomalies or chromosomal conditions; multiple pregnancies; advanced maternal age (35+); previous pregnancy loss or stillbirth; and IVF pregnancies. Being high-risk does not mean something will go wrong — it means closer monitoring is needed.
Do I need to leave my regular OB-GYN doctor to come to Mayflower?
No. Dr. Kunda Shahane works as a specialist consultant alongside your primary obstetrician — not instead of them. Her role is to provide advanced fetal surveillance and interpret complex findings. She does not take over your routine antenatal care. This collaborative model is standard in fetal medicine practice and ensures you receive the best of both specialist and routine care.
How often will I need scans in a high-risk pregnancy?
Frequency depends on your specific condition. Some patients need scans every 2–4 weeks for growth and Doppler monitoring; others with active IUGR, TTTS, or severe pre-eclampsia may need weekly surveillance. Dr. Kunda Shahane will plan an individualised monitoring schedule at your first appointment and adjust it as the pregnancy progresses.
Is fetal echocardiography necessary for every high-risk pregnancy?
Fetal echocardiography (fetal heart scan) is strongly recommended in all high-risk pregnancies — especially when the mother has pre-existing diabetes, thyroid disease, SLE, or a family history of congenital heart disease, when the anomaly scan raises a cardiac concern, and for all IVF pregnancies. At Mayflower, fetal echo is performed by Dr. Kunda Shahane using AI-assisted STIC technology on the GE Voluson Signature Expert.
I had a miscarriage before. Does that make my current pregnancy high-risk?
A single miscarriage does not automatically make a subsequent pregnancy high-risk in all cases. However, two or more consecutive pregnancy losses (recurrent pregnancy loss) warrants investigation and closer monitoring. Dr. Kunda Shahane can review your history, recommend any investigations needed, and establish an early monitoring schedule to provide reassurance and detect any problems as early as possible.
I have an abnormal NIPT result. What should I do?
An abnormal NIPT result requires prompt specialist counseling — not just a repeat NIPT. Dr. Kunda Shahane will review the specific NIPT finding, assess the ultrasound features, explain what the result means in your specific context, and discuss whether invasive testing (amniocentesis) is recommended to confirm the result. NIPT is a screening test — a positive NIPT does not automatically mean the baby has the condition. Dr. Kunda will help you understand your actual risk and next steps.
Can I get a second opinion if I received a worrying scan result elsewhere?
Yes, and many patients come to Mayflower specifically for this reason. Dr. Kunda Shahane will review all previous reports and scan images, perform a fresh assessment on the GE Voluson Signature Expert, and provide an independent specialist opinion. You do not need a referral letter to book a second-opinion appointment. Please bring all original scan images and reports.
Is ultrasound safe to have frequently during pregnancy?
Yes. Diagnostic ultrasound — including Doppler — has been used safely in pregnancy for over four decades. No adverse effects on the baby from standard diagnostic ultrasound have been documented in over 40 years of clinical use. In high-risk pregnancies, the benefit of detecting complications early is far greater than any theoretical concern from the scans themselves.
What is Doppler monitoring and why is it important in high-risk pregnancy?
Doppler monitoring measures blood flow velocity in the umbilical artery, middle cerebral artery (MCA), and uterine arteries using sound waves. In a high-risk pregnancy, abnormal Doppler results are often the first sign that the placenta is not functioning well, or that the fetus is beginning to compensate for reduced oxygen supply. Catching these changes early — before the baby is in acute distress — gives the obstetric team time to plan the safest management.
What documents should I bring to my high-risk pregnancy appointment?
Please bring: all previous ultrasound scan reports and images; blood test results (thyroid, glucose, blood group, NIPT if done); your antenatal card or booking summary; referral letter if available; current medications list; and any previous pregnancy records (discharge summaries, genetic reports) if relevant. A full bladder is generally not required — Dr. Kunda will advise specifically if it is needed for your type of scan.
"

A high-risk label in pregnancy is not a verdict — it is a signal that closer attention is needed. In my experience, most families who come to me after a worrying finding or a difficult previous pregnancy are dealing with two things at once: a medical problem and profound anxiety. My job is to address both. The best thing I can give a patient with a high-risk pregnancy is clarity — a clear understanding of what is actually happening, what we are watching for, and what we will do if things change. That clarity, supported by the right monitoring, is what allows families to move through a difficult pregnancy with knowledge rather than fear.
