What is fetal arrhythmia?

Fetal arrhythmia means that the fetal heartbeat is not following the expected regular pattern. It may be irregular, faster than expected, or slower than expected.

Some rhythm changes are temporary and may settle on their own. Others, especially persistent fast rhythm, persistent slow rhythm, suspected heart block, or rhythm change with fetal swelling, require detailed fetal echocardiography and close monitoring.

Irregular beats

Extra or skipped beats are often picked up during routine scans. Many are benign, but they should be documented and followed if persistent.

A fetal heart rhythm scan is usually advised when a routine ultrasound, Doppler, NST, or obstetric check-up suggests that the fetal heartbeat is irregular, unusually fast, unusually slow, or difficult to interpret.

How the diagnosis is made

Why M-mode and Doppler are important

In the fetus, we cannot place ECG leads directly on the baby like after birth. Ultrasound rhythm analysis therefore uses indirect but powerful methods: M-mode movement timing and Doppler blood-flow timing.
