Most women arriving at a gynaecology clinic do not have a diagnosis. They have a symptom — periods that have become heavy, pain that stops them working, bleeding that has appeared at the wrong time, a discharge that will not settle, a Pap result they do not understand, or a decision to make about contraception. Dr. Kunda Shahane qualified MBBS in 2004 and completed MS in Obstetrics & Gynaecology at Government Medical College, Nagpur. She then spent three years as Assistant Professor in Obstetrics & Gynaecology at Sumandeep Medical College before subspecialising in ultrasound and fetal medicine. Gynaecology is not something she does alongside fetal medicine — it is the training that came first and that everything else is built on.
That order matters in a practical way. When a scan shows a fibroid, a polyp, a thickened endometrium or a polycystic ovary pattern, the person who performed the scan is also the person who decides whether it explains your symptoms, whether it needs treating, and what it means for a pregnancy you may want later. There is no report to take to a second doctor and no gap between the imaging and the decision.

Periods and cycle problems
Cycles that have become irregular, absent, prolonged or unpredictable; periods heavy enough to cause anaemia; and pain that is not controlled by simple painkillers. Assessment starts with a history, an examination and a pelvic ultrasound in the same visit.
Abnormal bleeding
Bleeding after intercourse, between periods, after the menopause, or in pregnancy. Each of these has a different list of causes and a different urgency, and each needs the cervix looked at rather than the symptom simply reassured away.
Fibroids, polyps and endometriosis
A scan finding is the beginning of a decision, not the end of one. What matters is whether it is causing your symptoms, whether it is changing, and what your plans are for pregnancy. Some need surgery; many need medication or monitoring.

Cervical screening and HPV

No one method suits everyone. What fits is decided by your plans, your tolerance for bleeding changes and your daily life. Counselling covers hormonal and non-hormonal options, intrauterine devices, and permanent methods.

Vaginal and urinary health
Discharge, itching, burning and recurrent infection, examined and swabbed rather than treated blind; and urinary leakage, which is common after childbirth and around the menopause and is far more treatable than most women are told.
Sexual health and STI care
Confidential testing and treatment for sexually transmitted infections, and assessment of pain during intercourse, which usually has a physical cause that can be found on examination.
Menopause
Hot flushes, sleep disturbance, mood change, vaginal dryness and bone health, assessed together rather than symptom by symptom. Any bleeding after the menopause is investigated, never watched.
Fertility and pre-marriage counselling
First-line fertility assessment — cycle tracking, ovulation, tubal and uterine evaluation — with onward referral where IVF is indicated. Pre-marriage counselling covers contraception, vaccination and carrier testing before a pregnancy is planned.
The gynaecologist performs the scan
In most gynaecology pathways in Nagpur the consultation and the ultrasound happen in different places, on different days, with different people. You describe your symptoms to one doctor, are sent to a sonologist who does not know the history, and return with a report ending in a request to correlate clinically.
At Mayflower Clinic the ultrasound is part of the consultation. Dr. Kunda takes the history, examines you, and then scans with the question already in mind. That changes what is looked for and what is measured. Endometrial thickness means one thing on day six of a cycle and another on day twenty-two; a 3 cm fibroid matters enormously if it sits inside the cavity and very little if it sits on the outer wall; an ovary that looks polycystic on a scan is not by itself a diagnosis of PCOS. These are interpretation problems, not imaging problems, and they are why the scan and the consultation belong together.

What happens at a gynaecology consultation
- History, in your own wordsBring the dates of your last few periods if you can, any previous scan or blood reports, and a list of medicines you take. Nothing needs to be typed up — a photograph on your phone is enough.
- ExaminationAn abdominal and, where appropriate, an internal examination. What is being done and why is explained first, and you may ask for it to stop at any point.
- Ultrasound in the same roomTransabdominal, transvaginal, or both, depending on what is being looked for. You will be told what is being seen as it is being seen, rather than waiting for a report.
- Explanation and planWhat the findings mean, what the options are, and what happens if you do nothing — which is a legitimate option in more cases than most women expect.
- Tests, if neededBlood tests, swabs, a Pap smear or an HPV test may be arranged on the same day. Where a partner needs a test or treatment, it is arranged as part of the same plan.
- Follow-upA written summary, and a review appointment where the plan depends on how you respond. WhatsApp is used for report queries between visits.

Gynaecology and obstetrics are not separate departments in this practice. A woman seen for heavy periods at thirty may return for pre-pregnancy counselling at thirty-two, antenatal care and scanning at thirty-three, and contraception advice afterwards — with one person holding the history rather than four. Where a pregnancy becomes complicated, the fetal medicine expertise is in the same room rather than at the end of another referral.
